Complete this pre-task plan with the crew before starting any new task or when conditions change. It walks the foreman through breaking the work into steps, naming the hazards, and agreeing on controls so everyone starts the task with the same plan.

Define the task

• Describe the task in plain language and identify where it will be performed
• Break the task into major steps in the order they will be done
• Identify who is on the crew and confirm each worker is trained for their part
• Confirm the schedule, duration, and any coordination with other trades
• Identify tools, equipment, and materials needed and confirm they are on hand

Identify hazards

• Review each step for fall hazards, including edges, openings, and ladders
• Review for struck-by hazards from equipment, traffic, or falling material
• Review for caught-in hazards, including trenches, rotating parts, and pinch points
• Review for electrical hazards, including energized circuits and overhead lines
• Check for hazardous substances, dust, fumes, or confined space concerns
• Consider weather, heat, cold, and site conditions that could change during the task
• Ask the crew what could go wrong and record any hazards they raise

Assign controls

• Eliminate or engineer out hazards where possible before relying on PPE
• List the specific control for each hazard identified, step by step
• Confirm required PPE for the task and verify the crew has it
• Identify barricades, signage, or spotters needed to protect others nearby
• Verify fall protection equipment and anchor points where tie-off is required

Permits and preconditions

• Confirm required permits are issued, such as hot work, excavation, or confined space
• Verify utility locates are current before any digging or drilling
• Apply lockout and tagout where the task involves stored or electrical energy
• Confirm rescue and emergency procedures specific to this task

Crew review and sign-off

• Walk through the plan with the whole crew at the work location
• Confirm every crew member can state the top hazards and their controls
• Agree on stop-work authority and who to notify if conditions change
• Have each crew member sign or initial the plan before work begins
• Revisit and update the plan if the task, crew, or conditions change
