Use this checklist to prepare and run an effective toolbox talk, then capture attendance on the sign-in sheet below. A short, focused talk tied to the day's work is worth more than a long generic one, and the signed record is your proof of training.

Choose the topic

• Choose a topic relevant to today's tasks, recent incidents, or the season
• Check the training schedule so required topics are covered over the month
• Rotate topics so the same subjects are not repeated back to back
• Confirm the topic applies to the trades and subcontractors present this week

Prepare the talk

• Review the talk material and note the two or three points that matter most
• Gather any props, tools, or PPE examples that make the topic concrete
• Pick a time and location where the whole crew can hear without distraction
• Notify subcontractor foremen so their crews can attend
• Record the date, project, topic, and presenter on this form

During the talk

• State the topic and why it applies to today's work on this site
• Cover the key hazards and the specific controls the crew will use
• Point out where the topic applies on this site, walking to the location if practical
• Ask questions and let crew members share near misses or concerns
• Answer questions raised, or commit to a follow-up answer with a date
• Keep the talk short and focused, generally 10 to 15 minutes

After the talk

• Have every attendee print and sign their name on the sign-in sheet
• Note any workers absent who need a make-up briefing before starting work
• Record any hazards, questions, or action items raised during the talk
• Assign owners and due dates for each action item
• File the completed record with the project safety documentation
• Note the topic and follow-ups to carry into the next talk
